Trulicity doses: what's available

Trulicity comes in four once-weekly dose strengths. All are delivered using the same single-use auto-injector pen — the dose is preset; there is no dose dial on the Trulicity pen. You receive the specific dose based on which pen your prescription is written for.

Trulicity starting dose

What is the starting dose of Trulicity?

The standard starting dose of Trulicity is 0.75 mg once weekly by subcutaneous injection. This is the lowest available strength. For most patients, this dose is high enough to produce meaningful A1c improvement while minimizing the nausea that comes with starting a GLP-1 medication.

Can you start Trulicity at 1.5 mg instead?

Clinical guidelines allow starting directly at 1.5 mg in some circumstances — for example, a patient switching from another GLP-1 who is already adapted to the class. However, most US prescribers start at 0.75 mg for at least the first 4 weeks to reduce the intensity of initial GI side effects. Starting high is associated with higher rates of nausea and vomiting.

Trulicity dosing schedule and titration

Trulicity is titrated — meaning the dose is increased in steps over time — to balance glycemic benefit against GI side effects. A typical schedule looks like this:

  • Weeks 1–4: 0.75 mg weekly (starting dose).
  • Week 5 onward: If additional A1c reduction is needed and side effects are tolerated, increase to 1.5 mg weekly. This is the standard maintenance dose.
  • Week 9 or later: If 1.5 mg is not enough and has been tolerated for at least 4 weeks, increase to 3.0 mg weekly.
  • Week 13 or later: If 3.0 mg is not enough and has been tolerated for at least 4 weeks, increase to the maximum 4.5 mg weekly.

Not every patient needs to move through every step of the ladder. Many stay at 1.5 mg indefinitely with good glycemic control. The decision to titrate up is based on A1c response and tolerability.

When should I increase my Trulicity dose?

Your clinician decides when to increase. As a general rule, dose increases are considered when: (1) you have been at the current dose for at least 4 weeks, (2) side effects have settled to a tolerable level, and (3) your A1c has not yet reached your target. Do not increase your own dose by using a different pen strength on your own — all dose changes should be made by a prescriber.

Trulicity maximum dose

The maximum approved dose of Trulicity is 4.5 mg once weekly. This was added to the label in 2020 following the AWARD-11 trial, which showed that 3.0 mg and 4.5 mg doses produced additional A1c and weight reduction beyond the previous maximum of 1.5 mg.

Is there a Trulicity dose higher than 4.5 mg?

No. 4.5 mg is the highest FDA-approved weekly dose. Patients who need more glycemic control than 4.5 mg provides are typically evaluated for an add-on diabetes medication (basal insulin, an SGLT2 inhibitor) or for a switch to a different GLP-1 or dual GIP/GLP-1 agonist such as Mounjaro (tirzepatide), which has a higher ceiling.

Is the 4.5 mg dose worth it?

In AWARD-11, patients on 4.5 mg experienced an additional ~0.3–0.5% A1c reduction and about 1–2 kg (2–4 lb) additional weight loss compared to 1.5 mg, but GI side effects were also more pronounced. Whether the additional benefit is worth the additional side effects depends on your glycemic target, tolerability, and goals.

Trulicity dose for weight loss

A reminder: Trulicity is not FDA-approved for weight loss. Any weight change on Trulicity is a secondary effect of the medication's appetite-suppressing mechanism. Still, for patients who have type 2 diabetes and who are also hoping for modest weight loss, the dose-response is worth understanding.

What is the best Trulicity dose for weight loss?

Within the Trulicity label, the 4.5 mg weekly dose produces the largest mean weight loss — roughly 9–11 pounds versus about 4–6 pounds at 1.5 mg, on average over a year. That said, the differences between 1.5 mg, 3.0 mg, and 4.5 mg are modest, and side effects scale with dose. The "best dose" is the one your clinician has chosen based on your overall diabetes care, not a self-selected "weight loss dose."

Can I ask for a higher Trulicity dose to lose more weight?

You can have the conversation, but clinicians are generally cautious about titrating up purely for weight loss in patients whose diabetes is already well controlled on a lower dose. How to use the Trulicity pen

The Trulicity pen is designed to be one of the simplest GLP-1 injection devices on the market. The needle is hidden at all times, there is no dose dial, and the pen clicks to confirm when the injection starts and when it's complete.

Step by step

  1. Remove the pen from the refrigerator and let it sit at room temperature for a few minutes if you find cold injections uncomfortable.
  2. Choose a clean injection site on your abdomen, thigh, or (if someone else is administering) the back of your upper arm.
  3. Clean the site with an alcohol swab and let it dry.
  4. Pull the grey base cap straight off the pen. Do not replace the cap after removing it.
  5. Place the clear base of the pen flat against your skin. Make sure the base is in full contact with the skin — no gap.
  6. Press and hold the green button on the top of the pen. You will hear the first click — the injection has started.
  7. Keep the pen pressed against your skin for approximately 5–10 seconds. You will hear a second click when the injection is complete.
  8. Lift the pen away from your skin. Confirm that the grey plunger is visible through the window at the base of the pen, indicating the full dose has been delivered.
  9. Dispose of the entire pen in an FDA-cleared sharps container. Do not recap, reuse, or throw the pen in regular trash.

How long do you hold the Trulicity pen after the second click?

You can safely lift the pen off your skin any time after the second click. Eli Lilly's instructions recommend making sure you've heard both clicks and that the grey plunger is visible in the viewing window before removing the pen. Some patients find that waiting an extra 2–3 seconds after the second click gives them more confidence that the full dose has been delivered.

Does the Trulicity injection hurt?

Most patients describe the Trulicity injection as a brief pinch, less painful than a typical insulin injection because the needle is very thin and is deployed automatically. Some discomfort at the injection site — mild burning, redness, itching — can occur and usually resolves within minutes to hours. If you experience significant pain, swelling, or bruising that persists beyond a day, notify your clinician.

Trulicity injection sites

Trulicity is injected subcutaneously — into the fatty layer just under the skin, not into muscle. The FDA label identifies three approved injection sites:

  • Abdomen. The most commonly used site. Inject into the soft tissue of the abdomen, but avoid the 2-inch area directly around the navel. Abdomen absorption is generally consistent and reliable.
  • Front of the thigh. Use the middle portion of the front of the thigh, where there is enough subcutaneous fat for a clean injection. Avoid injecting too close to the knee or hip.
  • Back of the upper arm. This site is approved only when someone else administers the injection, because it's difficult to reach your own upper arm with good technique.

Where do most people inject Trulicity?

The abdomen is the most popular self-injection site because it is easy to see and reach, tends to have a stable layer of subcutaneous fat, and has consistent absorption. Rotating week to week — for example, right abdomen one week, left abdomen the next, thigh the third week — helps prevent injection-site reactions and lipohypertrophy over long-term use.

Can I inject Trulicity in my buttock?

The buttock is not an approved Trulicity injection site in the US label. Stick with the abdomen, thigh, or back of upper arm as specified in the patient instructions.

Best injection site for weight loss?

Is there a 'best' Trulicity injection site for weight loss?

No. Despite popular claims, there is no clinical evidence that injecting Trulicity in one approved site produces more weight loss than another. The pharmacokinetics — how quickly the drug is absorbed and how long it stays in the body — are similar across the abdomen, thigh, and upper arm. Social-media advice telling patients to inject in "the belly for more weight loss" is not supported by the FDA label or by clinical data.

What does matter for weight loss on Trulicity?

Adherence to the weekly schedule, diet, physical activity, sleep, and whether the dose is appropriate for your goals. Trulicity produces only modest weight loss on average (roughly 3–5% of body weight); if that's not enough for your goals, the biggest lever is a discussion with a clinician about whether a different medication class is appropriate.

Does Trulicity need to be refrigerated?

Yes — and the storage rules matter.

  • Standard storage: In the refrigerator at 36°F to 46°F (2°C to 8°C), in the original carton, protected from light.
  • Room temperature allowance: If needed, Trulicity can be kept at room temperature (not above 86°F / 30°C) for up to 14 days total — useful for travel.
  • Do not freeze. Discard any pen that has been frozen or exposed to freezing temperatures, even if it has thawed.
  • Do not shake the pen.
  • Protect from direct sunlight.

Can I travel with Trulicity?

Yes. For short trips (up to 14 days), Trulicity can be stored at room temperature provided it stays below 86°F. For longer travel, use an insulated medication travel case with ice packs — never pack Trulicity pens in checked luggage (where they can freeze) and never expose them to direct sunlight or hot car interiors. If you travel internationally, carry the pens in your original labeled packaging along with your prescription for border checks.

Missed or late Trulicity dose

What should I do if I miss a Trulicity dose?

If you remember the missed dose within 3 days (72 hours) of your scheduled injection day, inject the dose as soon as possible and then resume your normal weekly schedule from that point — either continue on the new day or go back to your original day at the next cycle, whichever your clinician prefers. If more than 3 days have passed, skip the missed dose and take your next dose on the regular schedule. Do not double up.

Can I change my Trulicity injection day?

Yes, with one rule: the day between two consecutive doses must be at least 3 days. For example, if you normally inject on Monday and want to move to Friday, you can inject on Friday as long as your last Monday dose was at least 3 days earlier.

What happens if I take Trulicity twice in one week by accident?

Accidentally injecting a second Trulicity dose within a few days of the scheduled dose can intensify GI side effects (nausea, vomiting) and, in patients on insulin or sulfonylureas, increase the risk of low blood sugar. Contact your clinician or pharmacist, hydrate well, and monitor blood glucose closely. Do not take the next scheduled weekly dose until at least 3 days after the most recent injection.

Does Trulicity work better at certain times of day?

No — Trulicity is a long-acting medication with a ~5-day half-life, which means drug concentrations are roughly steady throughout the week. The time of day you inject does not meaningfully affect how it works. Pick a consistent day that fits your routine. The day can be changed if needed, as long as the last injection was given at least 3 days earlier.

Does it matter if I inject Trulicity with or without food?

No. Trulicity can be injected with or without food and does not need to be timed around meals. Some patients who experience mid-week nausea find that the timing of the weekly injection in relation to meals on injection day is irrelevant; the nausea is driven by the ongoing GLP-1 effect, not the injection event itself.

Can the Trulicity pen be used for other medications?

No. The Trulicity pen is single-use and designed specifically for one weekly dose of dulaglutide. It is not refillable and should not be used to administer any other medication. After use, dispose of the entire pen in an FDA-cleared sharps container.
